For parents driving students on trips

Parents are now required to fill out a "Parental Transport Release Permission Form" or "Release of Liability to Transport multiple students form" to transport their own children or multiple students to or from a school sponsored event. These forms must be signed by the band director and by a school administrator, then returned to the band director at least 24 hours prior to the event. Note:  this does not apply to the Wilson and Milne practices!

Chaperone Background Checks

To apply to volunteer and complete your background check, follow these steps:

  1. Print a copy of the Volunteer Confidentiality Form and get school Principal’s signature verifying the request.
  2. Take the form, a picture ID and either a $18 cashier's check, money order, or credit card (NO personal checks or cash) to the APS Background & Fingerprinting Office.
  3. The clearance certificate will be mailed directly to the school.
  4. Volunteers who have cleared their background check will be given a volunteer card distributed by the school.
  5. All Chaperones must take the online course “Life of an Athlete”.  Once completed please print and turn-in Life of an Athlete Certificate to our Band Chaperone chairperson Sandra Montoya.

**Volunteers will be required to present this card each time they volunteer for a school activity.**

Band Letter

The purpose of a band letter is to encourage and acknowledge “above and beyond” accomplishments.  Because you are in band does not guarantee a band letter.  A letter must be earned and it shows extra dedication and effort above what is required for the regular band grades. Letters and other awards are given out at the annual Band Banquet held in May. DOWNLOAD HERE
